Evaluation of Patient Retention in The Methadone Maintenance Treatment Program at Embung Fatimah Hospital, Batam

Abstract
The Methadone Maintenance Therapy Program (MMTP) is a pharmacological harm-reduction intervention designed for opioid-dependent people who inject drugs. Patient retention serves as the principal indicator of MMTP success because it is directly related to reductions in risky behaviour, HIV transmission, and opioid-related mortality. This study aimed to evaluate the retention rate and methadone-dosing profile of the MMTP at Embung Fatimah Hospital, Batam, during January-December 2025. A descriptive observational retrospective design was applied to the historical cohort of patients ever registered in the MMTP (n=41); with active retention as consistent methadone dose receipt without absences of ≥14 consecutive days. Data were drawn from daily methadone dosing forms, monthly methadone usage reports, and the MMTP coordinator's recapitulation, and were analysed descriptively with independent cross-verification. The active retention rate was 4.88% (2 of 41 patients); both retained patients were males in the 25–49 years age category. Mean maintenance doses were 72.1 mg/day (range 65.0-82.5) for Patient 1 and 50.5 mg/day (range 45.2-56.7) for Patient 2, whereas total program consumption reached 40,930 mg over eleven months (mean 3,721 mg/month). Stock management with batch B40612J (expiry February 2027) was adequate. The retention level was markedly below the Ministry of Health's effective threshold (>55%) and the Indonesian average (60-65%), although both retained patients received doses within the effective range. Further investigation of historical cohort dynamics and hospital-level retention strategies is therefore warranted
